Rho has been uninhabited for most of history, except for the one Greek patriot who made it famous, Despina Achladioti. Despina Achladioti was a Greek patriot born on the island of Kastellórizo in 1890 Every day, Achladioti, also known as the Lady of Rho, would fly a Greek flag, clearly visible from Turkish soil, over the island. This act of patriotism, which made Despina famous around Greece, ended in 1982 with her death. Patriotism is commonly defined as love of and/or devotion to one's country

A small Greek military unit is now based on the island, with the primary duty of continuing the tradition of raising the flag, set by Achladioti. The 2001 census reported a population of 15 inhabitants. It is part of the municipality of Megísti (Kastelorizo). " Castellorizon " is also the name of a song from David Gilmour's album On an Island, based on a night he spent on Kastelorizo in 2006

The islet is named in Turkish Kara Ada. Turkish ( tr Türkçe IPA) is a language spoken by over 63 million people worldwide making it the most commonly spoken of the Turkic languages.
