The highway begins at the Iowa state line. Eight miles south of the Iowa state line at Unionville, Route 5 intersects with U.S. Route 136, with which it overlaps for one mile before turning south towards Milan. Beginning in the summer of 2008, MoDOT will begin a project to convert Route 5 into a "shared four-lane" highway between Lebanon and Camdenton.
