Diabetic foot is an umbrella term for foot problems in patients with diabetes mellitus. Diabetes mellitus (ˌdaɪəˈbiːtiːz or /ˌdaɪəˈbiːtəs/ /məˈlaɪtəs/ or /ˈmɛlətəs/ often referred to simply as diabetes ( Ancient Greek: grc Due to arterial abnormalities and diabetic neuropathy, as well as a tendency to delayed wound healing, infection or gangrene of the foot is relatively common. Arteries are Blood vessels that carry blood away from the Heart. Diabetic neuropathies are neuropathic disorders that are associated with Diabetes mellitus. An infection is the detrimental Colonization of a host Organism by a foreign Species. Prevention

Prevention is by frequent chiropody review, good foot hygiene, diabetic socks and shoes, and avoiding injury. Podiatry or podiatric medicine is a field of Healthcare devoted to the study and treatment of disorders of the Foot, Ankle, and the "anatomical A diabetic sock is a Sock which is specially designed for those with Diabetes. Diabetic shoes, sometimes referred to as extra depth or therapeutic shoes are specially designed shoes or inserts intended to reduce the risk of skin breakdown in diabetics

All major reviews recommend special footwear for patients with a prior ulcer or with foot deformities. One review added neuropathy as an indication for special footwear. The comparison of custom shoes versus well-chosen and well-fitted athletic shoes is not clear.
A meta-analysis by the Cochrane Collaboration concluded that "there is very limited evidence of the effectiveness of therapeutic shoes" [2]. In Statistics, a meta-analysis combines the results of several studies that address a set of related research hypotheses The Cochrane Collaboration is a group of over 11500 volunteers in more than 90 countries who apply a rigorous systematic process to review the effects of interventions tested in biomedical The date of the literature search for this review is not clear. Clinical Evidence reviewed the topic and concluded "Individuals with significant foot deformities should be considered for referral and assessment for customised shoes that can accommodate the altered foot anatomy. In the absence of significant deformities, high quality well fitting non-prescription footwear seems to be a reasonable option" [3]. National Institute for Health and Clinical Excellence has reviewed the topic and concluded that for patients at "high risk of foot ulcers (neuropathy or absent pulses plus deformity or skin changes or previous ulcer" that "specialist footwear and insoles" should be provided [4]

The one randomized controlled trial that showed benefit of custom foot wear was in patients with a prior foot ulceration [5]. A randomized controlled trial (RCT is a type of scientific Experiment most commonly used in testing the Efficacy or Effectiveness of Healthcare In this trial, the number needed to treat was 4 patients. The number needed to treat (NNT is an epidemiological measure used in assessing the effectiveness of a health-care intervention typically a treatment with Medication

Treatment

Foot ulcers in diabetes require multidisciplinary assessment, usually by diabetes specialists and surgeons. In Medicine, a surgeon is a person who performs Surgery. Surgery is a broad category of invasive Medical treatment that involves the cutting of a Treatment consists of appropriate bandages, antibiotics (against staphylococcus, streptococcus and anaerobe strains), debridement and arterial revascularisation. In modern usage an antibiotic is a Chemotherapeutic agent with activity against Microorganisms such as Bacteria, fungi or Protozoa Staphylococcus (in Greek σταφυλη staphyle means bunch of grapes and κοκκος coccos means granule is a genus of Gram-positive Streptococcus is a Genus of spherical Gram-positive bacteria belonging to the phylum Firmicutes and the An anaerobic organism is any Organism that does not require Oxygen for growth and may even die in its presence Debridement is a medical term referring to the removal of Dead, damaged or Infected tissue to improve the healing potential of the remaining

It is often 500 mg to 1000 mg of flucloxacillin, 1 g of amoxicillin and also metronidazole to tackle the putrid smelling bacteria.

Specialists are investigating the role of nitric oxide in diabetic wound healing. Nitric oxide or nitrogen monoxide is a Chemical compound with Chemical formula N[[Oxygen O]] [6] Nitric oxide is a powerful vasodilator, which helps to bring nutrients to the oxygen deficient wound beds. Specialists are using forms of light therapy such as LLLT to treat diabetic ulcers. Light therapy or phototherapy consists of exposure to daylight or to specific wavelengths of light using Lasers, LEDs Fluorescent lamps,

In 2004, The Cochrane review panel concluded that for people with diabetic foot ulcers, hyperbaric oxygen therapy reduced the risk of amputation and may improve the healing[7] at 1 year. The Cochrane Library is a collection of databases in medicine and other healthcare specialties provided by the Cochrane Collaboration and other organisations Hyperbaric medicine, also known as hyperbaric oxygen therapy (HBOT is the medical use of Oxygen at a higher than Atmospheric pressure. Amputation is the removal of a body extremity by trauma or Surgery. [8] They also suggest that the availability of hyperbaric facilities and economic evaluations should be interpreted. [8]
